Nurse Navigator - Breast Health

About this position

The Breast Health Nurse Navigator provides care coordination and support to customers undergoing breast diagnostic or treatment services. Responsibilities include: customer education, customer advocacy, support service coordination, and referral to next level of care. The Breast Health Nurse Navigator also evaluates customer's psychosocial needs, serves as a breast health liaison for the community, coordinates interdisciplinary care conferences, and participates in outcome analysis and required data tracking for MQSA and FDA regulations. Customers receiving care would be adult and geriatric customers.

 

What we’ll expect from you

• Manages customers and coordinates care plan activities through the breast health diagnostic evaluation process.
• Provides customer call backs for additional diagnostic imaging services.
• Demonstrates strong interpersonal skills by establishing collaborative relationships with all members of the health care team.
• Collaborates with physicians and other disciplines to provide comprehensive clinical coordination of care including the: Primary Care physicians, Radiologists, Surgeons, Plastic Surgeons, and Oncologists, and Nursing to develop the plan of care.
• Implements the Caring Framework for Customer Care defined by NMHC nurses and customers and families.
• Educates customer and family about the diagnostic and surgical interventions to ensure quality customer care delivery and continuity of care.
• Demonstrates the ability to provide support and care coordination to customers and families. Attends customer appointments for referral to next level of care if requested.
• Evaluates customer psychosocial needs. Provides referrals to customers who require social services and works with physicians to ensure the customer's psychological needs are met.
• Creates educational and resource information in collaboration with education department for customers in regard to breast health.
• Assists in coordinating weekly interdisciplinary conferences with physicians and members of care team.
• Maintains clinical knowledge of Breast Health to ensure the program is based on evidence and best practice.
• Serves as a breast health resource for the organization and community. Provides community education on breast health and breast cancer as a NMHC representative at community fairs and exhibits.
• Participates in multidisciplinary educational programs for the medical community regarding breast health, breast cancer detection, treatment, and care for the breast health customer and their families/ significant others.
• Participates and monitors customer follow up and outcome data analysis for MQSA and FDA regulations.
• Monitors customers who are experiencing adverse reactions to procedures as ordered by the radiologist or surgeon.
• Assists the clinic radiologist/surgeon in procedures performed in the Breast Center.
• Accepts other duties as assigned to promote the accomplishment of organizational goals.

Education

• Baccalaureate degree in nursing required.
• Must attend the Breast Health Navigator Certification course given by Educare within one year of time of hire.   

Experience, Knowledge, Skills, Abilities 

• One year of experience in breast health or three years as a breast cancer nurse required. • Demonstrated ability to manage significant customer population on a weekly basis.
• Demonstrated broad clinical knowledge of breast health and disease.
• Strong human relation skills.      

Licensure/Certification(s) Mandatory

• Current licensure as RN in State of Minnesota

• Current BLS Certification - State Requirement
• Must become certified as an oncology nurse through the Oncology Nurse Certification Corporation within three years of date of hire and maintain certification throughout employment of this position.
