About this position
Provides hospice customers and their loved ones with the appropriate spiritual/existential care at the end-of-life. The position supports hospice recipients through direct counsel, assessment of spiritual/existential needs, consultation with hospice team members, and by facilitating access to community-based resources.
Essential Duties and Responsibilities:
• Acts as a supportive presence in response to assessed hospice customer/family spiritual, existential, relational, and emotional needs • Assumes a counselor stance that demonstrates respect and curiosity toward customer/family beliefs • Completes spiritual assessment that identifies the preferred spiritual/religious/cultural/existential practices of hospice customer/family • Shares appropriate information about family dynamics, spiritual, religious, existential, and cultural needs with the hospice interdisciplinary team • Directly provides or facilitates the provision of rites and rituals that are requested and supportive to the hospice customer/family. • Makes appropriate referrals for customer/family to access community-based resources as needed. • Meets with hospice customer/family in their home environment • Responds in a timely manner when receiving requests for Spiritual Counselor services. • Consults with hospice interdisciplinary team regarding customers/families even when customer is not receiving Spiritual Counselor visits • Performs the required number of daily customer visits to meet the needs of the caseload. • Completes required hospice documentation of customer care and care coordination following Centers for Medicare & Medicaid Services, and North Memorial Hospice requirements including documentation of a care plan, visit sets, and all visits. • Participates in hospice Spiritual Counselor on-call rotation that serves customer/family needs. • Attends hospice Interdisciplinary Team meetings. • Accepts other duties as assigned to promote the accomplishment of organizational goals. • The team member is accountable to demonstrate proficiency for the skills outlined in the appropriate position skills list. • The team member is accountable to maintain skill proficiency, including improvement where deemed necessary, and upgrading any additional or new skills on the appropriate position skills list. • Recognizes and complies with legal, regulatory, accrediting and procedural requirements related to area of responsibility. • Understands and follows principles and standards as outlined in North Memorial's Corporate Code of Conduct. • Protects customer and team member privacy and only accesses customer and/or team member related information as needed to perform job duties. • Reports violations or areas of concern to supervisor or Corporate Compliance Officer via established methods of communication.

Requirements
Education
• Bachelor's degree required; graduate level in theological studies or related field preferred. • Sensitive to spiritual pluralism and able to provide for the free exercise of spirituality by all customers. • Completed four units (1,600 hours) of clinical pastoral education (CPE), preferred.
Experience
• At least two years of spiritual counseling experience preferred. • Experience with death and dying preferred
Licensure/Certification
Valid Class D driver's license in state of residency with acceptable driving record, and annual proof of auto insurance per hospital policy - BH Standard
