Community Health Specialist, Sr

Robbinsdale, MN

About this position

The Community Health Specialist, Senior partners with Community and Executive leaders to develop, lead and project manage the execution of strategic programs that can improve the health of the community with high-value solutions and sustainable results. The Senior Specialist will serve as a liaison to the community and NMH partners while leveraging strong project management, process improvement and measurement methodologies. This position is also responsible for project managing the community health assessment every three years as well as the execution of the implementation plans each year.
BS/BA degree required, preferably in a health care related field.

 

What we’ll expect from you

Accountabilities:
• Responsible for leading project management for Community Health programs, including defining goals, measurements, deliverables, key milestones. This position will require proven experience in managing and measuring programs to demonstrate results.
• Develops relationships with organizations, agencies, and members of the community.
• Works with community organizations, insurance partners and internal teams and departments to build, execute and maintain programs that improve the health of our customers and the community.
• Establishes and/or implements innovative partnerships and helps build collaboration both internally and externally. This includes coordinating activities – that engage the community – with internal and external entities like non-profit agencies, government, and business groups.
• Partners with Director of Marketing and Outreach and senior leaders to seek alignment of initiatives across the health system. Collaborates across the organization, building relationships and driving the work through to completion.
• Identify and apply best practices from within and outside of North Memorial Health to ensure the best programs. Learn each year and refine as necessary to continue to meet goals.
• Works closely with hospital administrators, system, other Community Health leaders, and key community partners to ensure Implementation Plans are completed and executed.
• Partner with and integrate into the Marketing and Outreach team to effectively see how other projects/programs/efforts connect and leverage those synergies to provide the most value for the organization and create the most impact in the community.
• Effectively applies process improvement to guide the community health work, including the assessment process.
• Supports Director of Marketing and Outreach an Consultants in developing the community health assessment, including project management, serving as a liaison with research partners, vendors, etc.
• Supports hospital and outreach leadership by participating in and leading various ad-hoc projects on an as-needed basis.
• Collaborates with system leaders and the Director of DE&I to drive the engagement with and health of underserved communities (social determinants of health).
• Drives key deliverables like presentations, summary reports, program summaries, measurement and status reports.
• Partners across the organization to maintain CBISA tracking records annually.
• Accepts other duties as assigned to promote the accomplishment of organizational goals.
• The team member is accountable to demonstrate proficiency for the skills outlined in the appropriate position skills list.
• The team member is accountable to maintain skill proficiency, including improvement where deemed necessary, and upgrading any additional or new skills on the appropriate position skills list.

Compliance:
• Recognizes and complies with legal regulatory, accrediting and procedural requirements related to area of responsibility.
• Understands and follows principles and standards as outlined in North Memorial’s Corporate Code of Conduct.
• Protects customer and team member privacy and only accesses customer and/or team member related information as needed to perform job duties.
• Reports violations or areas of concern to supervisor or Corporate Compliance Officer via established methods of communication.

Education

BS/BA degree required, preferably in a health care related field.   

Experience, Knowledge, Skills, Abilities 

• Minimum of 5 - 7 years experience in community outreach/community health
• Experience collaborating across teams to build and execute lasting community health programs
• Ability to leverage community health data to drive program development
• Experience serving as a liaison to communities and partners
• Ability to communicate within teams, committees and with senior leaders
• Tracking and measuring all work to show program effectiveness
• Experience working on programs that address social determinants of health

Knowledge, Skills and Abilities:
• Strong project management skills
• Ability to manage complex projects from planning to implementation
• Strong relationship skills
• Excellent written and verbal communications skills, developing reports, etc.
• Ability to manage multiple projects concurrently
• Strong presentation skills
• Ability to think creatively and develop programs that meet community health needs in new, inventive ways
