Quality Improvement Specialist

About this position

Provides support to division and department management and medical staff to improve clinical outcomes
of a variety of customer populations. Facilitates clinical improvement projects, assists with improvement
measurement and analysis, and assists with reporting of clinical outcomes and improvements. Integrates
improvement support activity with related regulatory and accreditation standards to promote compliance

 

What we’ll expect from you

• Utilizes quality improvement techniques to improve customer care and clinical outcomes.
• Assists with standardizing customer care, incorporating evidence based practices and clinical care pathways.
• Emphasizes alignment of improvement initiatives with NMHC strategic objectives; assists in coordinating scorecard measures with strategic initiatives.
• Assists with improving measurement and analysis, assists with reporting of clinical outcomes and improvements.
• Incorporates regulatory accreditation requirements into improvement projects.
• Abstracts data from medical records for key clinical improvement projects.
• Collaborates with Improvement Systems to recognize improvement team accomplishments and goals achieved.
• Acts as a resource to identify trends in healthcare quality improvement, incorporates into planning and development of improvement initiatives, and maintains current knowledge of regulation and standards.
• Assists with identifying benchmarks and comparisons for clinical outcomes measures.
• Collaborates with Registry Coordinators to ensure consistent analysis and data integrity for external reporting.
• Advises and assists leadership on matters affecting quality and improvement.
• Accepts other duties as assigned to promote the accomplishment of organizational goals.

• The team member is accountable to maintain skill proficiency, including improvement where deemed necessary, and upgrading any additional or new skills on the appropriate position skills list.

Education

• Bachelor's Degree in nursing or a health care related field. Significant related Quality experience may offset degree.
• Home and Community: Associate degree and 1 year professional level home health quality improvement experience may be substituted for Bachelor's Degree   

Experience, Knowledge, Skills, Abilities 

• Three or more years previous health care experience required
• Direct quality improvement experience preferred
• Home and Community only: Two or more years' experience in a Medicare certified home health agency

• Demonstrated verbal, written and formal presentation skills.
• Demonstrated leadership skills, including but not limited to team leading, supervision, charge or management position preferred.
• Demonstrated competence with computer skills including Microsoft Windows operating system and office tools.
• Home and Community only: Knowledge of home health and hospice Medicare conditions of participation OASIS, OBQI, OBQM, and QAPI outcome measures preferred      

Licensure/Certification(s) Preferred

• Certification or license in health care- - NMHC Preferred
• OASIS certification - NMHC Preferred
